Juniper Cove Venue Disc Golf Course (JCV DGC) WAIVER AND RELEASE

In consideration of being allowed to participate on the Juniper Cove Venue Disc Golf Course (JCV DGC hereafter), and any and all related events and activities as a, including but not limited to, *player, player’s caddie, player’s support personnel, staff member, media member, volunteer, vendor, or spectator, the undersigned hereby acknowledges and agrees how:

1. I acknowledge and fully understand how my participation at JCV DGC may involve the risk of injury, including the possibility of serious injury or death, not only from my own actions, but also from the action or inaction of others, the condition of the premises, or the failure of the equipment used. I knowingly agree to assume all such risk, both known and unknown assuming full responsibility for my participation. I acknowledge how JCV DGC does have a lake (Sydney) and small creek among it’s features, and any entrance into said waters is at the discretion of any attendees and are subject to the same rules. NOTE: Sydney Lake is part of Flat Rock Estates Sub-division which is not liable for any issues regarding JCV DGC participants.

2. I agree to conduct myself during time at JCV DGC with all due regard for my own safety and same for all other participants, observers and bystanders. I agree to comply with the directions of JCV DGC management, administrators, or proxy personnel, and I agree to immediately notify JCV DGC personnel if I observe any unsafe condition or behavior during this time.

3. For myself, and on behalf of all of my heirs, assigns, executors, personal representatives and next of kin, I hereby waive and forever release the officers, directors, agents, volunteers, sponsors, donors, or local organizers of any organized events at JCV DGC from and against any and all liabilities, claims, expenses or causes of action of any kind or nature whatsoever, arising out of or relating to my participation.

4. I both recognize and understand how my presence, or *those with me during the intended time period, MUST have individually completed this WAIVER and RELEASE agreement prior to physically entering onto JCV grounds or else my/their presence would be considered trespassing and then be liable for any subsequent legal or injurious actions thereafter.

I have read this WAIVER and RELEASE agreement, fully understand its terms, and understand how I have given up substantial rights by signing it, and I sign it freely and voluntarily without any inducements.

Furthermore, I also recognize JCV DGC personnel, or event organizers, have the right and responsibility to record said event through video and still photography of any and all participants. I hereby grant permission how such photography may be used for event

publicity, public information, or any other promotional use which falls within JCV or event host activity.
